
LPN - Home Health
Providence St. Patrick Hospital is seeking a compassionate and skilled LPN to join our dynamic Home Health team. As an LPN in our Home Health program, you will have the opportunity to make a meaningful impact on the lives of our patients by providing high-quality, personalized care in the comfort of their own homes. - Provide compassionate and skilled care to patients in the Home Health program.
- Administer medications and treatments as prescribed by physicians.
- Monitor and assess patient health status, report any changes or concerns to the physician.
- Develop and implement individualized care plans for patients in collaboration with the interdisciplinary team.
- Educate patients and their families on disease management, treatment plans, and medication administration.
- Communicate effectively with patients, families, and other healthcare professionals.
- Ensure accurate and timely documentation of patient care in electronic medical records.
- Adhere to all state and federal regulations and company policies.
- Maintain a high level of professionalism and ethical standards in all interactions.
- Collaborate with other members of the Home Health team to provide comprehensive care to patients.
- Participate in ongoing training and development to maintain and enhance skills and knowledge.
- Identify and address any safety concerns in the patient's home.
- Maintain a clean and organized work environment.
- Continuously evaluate and improve processes to enhance the quality of patient care.
- Support and promote a positive and inclusive work environment.
Strong Communication And Interpersonal Skills.
Current Lpn License In The State Of Montana.
Minimum Of 1 Year Experience Working In Home Health Or A Related Field.
Ability To Work Independently And Manage Time Effectively.
Knowledge Of Medicare/Medicaid Regulations And Documentation Requirements.
