LPN - Home Health

Providence St. Patrick Hospital is seeking a compassionate and skilled LPN to join our dynamic Home Health team. 1. Provide compassionate and skilled care to patients in the Home Health program.
2. Administer medications and treatments as prescribed by physicians.
3. Monitor and assess patient health status, report any changes or concerns to the physician.
4. Develop and implement individualized care plans for patients in collaboration with the interdisciplinary team.
5. Educate patients and their families on disease management, treatment plans, and medication administration.
6. Communicate effectively with patients, families, and other healthcare professionals.
7. Ensure accurate and timely documentation of patient care in electronic medical records.
8. Adhere to all state and federal regulations and company policies.
9. Maintain a high level of professionalism and ethical standards in all interactions.
10. Collaborate with other members of the Home Health team to provide comprehensive care to patients.
11. Participate in ongoing training and development to maintain and enhance skills and knowledge.
12. Identify and address any safety concerns in the patient's home.
13. Maintain a clean and organized work environment.
14. Continuously evaluate and improve processes to enhance the quality of patient care.
15. Support and promote a positive and inclusive work environment.
