
Care Manager: Population Health

- Develop and implement strategies to improve population health outcomes in the community.
- Collaborate with other healthcare professionals to identify and address gaps in care for specific populations.
- Conduct comprehensive assessments of patient needs and develop individualized care plans.
- Monitor and track patient progress and outcomes, making necessary adjustments to care plans as needed.
- Utilize evidence-based practices and data analytics to drive decision-making and improve care processes.
- Serve as a liaison between patients, healthcare providers, and community resources to ensure continuity and coordination of care.
- Educate and empower patients to take an active role in their own healthcare management.
- Communicate with insurance companies and other payers to ensure appropriate coverage and reimbursement for services provided.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Stay current on industry trends and best practices in population health management.
- Maintain accurate and up-to-date documentation of patient interactions and interventions.
- Demonstrate a commitment to the organization's mission, values, and goals.
- Act as an advocate for patients and their families, addressing any concerns or barriers to care.
- Serve as a mentor and resource for other healthcare professionals, promoting a collaborative and supportive team environment.
- Adhere to all regulatory and compliance standards to ensure a safe and ethical work environment.
Bachelor's Or Master's Degree In Nursing, Public Health, Or Related Field.
Minimum Of 3-5 Years Of Experience In Healthcare Management, Preferably In Population Health Or Care Coordination.
Strong Understanding Of Population Health Strategies, Data Analytics, And Care Management Processes.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Multidisciplinary Teams And Communicate Complex Information To Patients And Providers.
Knowledge Of Healthcare Regulations And Policies, Including Medicare And Medicaid Guidelines, As Well As Experience With Electronic Health Records And Care Management Software.
