  1. Provide exceptional pharmaceutical care to patients in an outpatient setting.
  2. Develop and implement medication management strategies to improve patient outcomes and promote wellness.
  3. Conduct medication therapy management and patient education to ensure safe and effective use of medications.
  4. Collaborate with healthcare providers to optimize medication therapy and manage patients with complex medical conditions.
  5. Maintain accurate and complete patient records and documentation.
  6. Keep up-to-date with the latest developments in pharmaceuticals and healthcare to provide the best possible care to patients.
  7. Adhere to all relevant laws, regulations, and standards related to pharmacy practice.
  8. Participate in quality improvement initiatives to enhance patient care and safety.
  9. Provide support and guidance to pharmacy technicians and other team members.
  10. Foster a positive and collaborative work environment within the pharmacy team.
  11. Continuously assess and improve upon processes and procedures to increase efficiency and effectiveness.
  12. Maintain a high level of professionalism and ethical standards in all interactions with patients, colleagues, and other healthcare professionals.
  13. Serve as a resource for patients, healthcare providers, and other team members regarding medication-related inquiries and issues.
  14. Act as an advocate for patient's health and wellness by identifying and addressing medication-related problems.
  15. Participate in training and development opportunities to enhance skills and knowledge.
  16. Adhere to company policies and procedures, including those related to safety and confidentiality.
  17. Contribute to the overall success of the pharmacy department and organization.
  18. Represent Sutter Health in a positive and professional manner at all times.

Job Qualifications
  • Active Pharmacist License In The State Of California.

  • Doctor Of Pharmacy (Pharm.d.) Degree From An Accredited College Of Pharmacy.

  • Completion Of A Post-Graduate Residency Program In Ambulatory Care Or Equivalent Experience.

  • Strong Clinical Knowledge And Experience In Managing Chronic Diseases, Such As Diabetes, Hypertension, And Asthma.

  • Excellent Communication And Interpersonal Skills To Collaborate With Healthcare Providers And Educate Patients On Medication Management.
