- Provide emotional and psychological support to patients and their families during end-of-life care.
- Offer practical assistance and resources to patients and families in navigating the hospice process.
- Conduct thorough assessments of patients' psychosocial needs and develop individualized care plans.
- Collaborate with interdisciplinary team members to ensure holistic and coordinated care for patients.
- Facilitate family meetings and discussions to address concerns and facilitate decision-making.
- Advocate for patients' rights and preferences in their end-of-life care.
- Monitor and assess patients' mental health and provide appropriate interventions and referrals as needed.
- Maintain accurate and up-to-date documentation of patient care and progress.
- Provide education and support to patients and families on coping mechanisms and self-care during end-of-life care.
- Participate in interdisciplinary team meetings and contribute to care planning and coordination.
- Stay informed about current hospice care practices and regulations.
- Demonstrate empathy, compassion, and sensitivity towards patients and their families throughout the end-of-life process.
- Uphold Sutter Health’s values and commitment to patient-centered care.
- Maintain confidentiality and adhere to ethical standards in all interactions with patients and families.
- Collaborate with community resources and organizations to provide additional support and services to patients and families.
Master's Degree In Social Work (Msw) From An Accredited Program.
Valid State License As A Licensed Clinical Social Worker (Lcsw).
Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Hospice Or Palliative Care.
Knowledge And Understanding Of The Psychosocial And Emotional Needs Of Terminally Ill Patients And Their Families.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams And Provide Compassionate Support To Patients And Their Loved Ones.
