Job Description

As our Community Support Specialist, you will play a vital role in providing individuals with the resources and guidance they need to thrive.   1. Provide direct support to individuals with mental health challenges in the community.
  2. Conduct assessments to identify individuals' needs and develop individualized support plans.
  3. Coordinate and connect individuals to community resources, including but not limited to housing, employment, healthcare, and social services.
  4. Collaborate with other members of the treatment team to ensure comprehensive care for individuals.
  5. Act as an advocate for individuals and assist them in navigating systems and services.
  6. Educate individuals and their families about mental health, community resources, and self-care strategies.
  7. Monitor and document progress towards goals and adjust support plans accordingly.
  8. Provide crisis intervention and de-escalation support when necessary.
  9. Maintain confidentiality and adhere to ethical standards in all interactions.
  10. Participate in team meetings, trainings, and other professional development opportunities.
  11. Collaborate with community partners to expand and enhance resources for individuals.
  12. Maintain accurate and timely documentation of services provided.
  13. Uphold the mission and values of Thresholds in all interactions and activities.
  14. Participate in program evaluation and quality improvement efforts.
  15. Demonstrate cultural competence and sensitivity in working with individuals from diverse backgrounds.
  16. Handle any other tasks and responsibilities assigned by the supervisor.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or A Related Field.

  • Excellent Communication And Interpersonal Skills.

  • Previous Experience Working With Individuals With Mental Health Diagnoses.

  • Strong Understanding Of Community Resources And Ability To Connect Clients To Appropriate Services.

  • Ability To Work Independently And As Part Of A Team To Provide Comprehensive Support To Clients.
