  1. Provide compassionate care coordination for members by developing and implementing personalized care plans.
  2. Conduct comprehensive assessments of members' health needs and collaborate with healthcare professionals to ensure the delivery of appropriate services.
  3. Communicate effectively with members, their families, and healthcare providers to ensure continuity of care.
  4. Maintain accurate and timely documentation of members' progress and treatment plans.
  5. Advocate for members' needs and ensure they receive appropriate and quality care.
  6. Identify and address any barriers to care and work to resolve them.
  7. Collaborate with internal teams and external partners to ensure the best possible outcomes for members.
  8. Stay up-to-date on industry regulations and best practices to provide the highest level of care to members.
  9. Use critical thinking and problem-solving skills to address complex healthcare situations.
  10. Demonstrate empathy and understanding towards members and their unique needs.
  11. Prioritize and manage multiple tasks and responsibilities in a fast-paced environment.
  12. Uphold the values and mission of UnitedHealth Group in all interactions and decisions.
  13. Participate in team meetings and contribute to the continuous improvement of processes and procedures.
  14. Adhere to all privacy and confidentiality regulations and maintain strict confidentiality of members' information.
  15. Continuously seek ways to improve and enhance the quality of care for our members.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Nursing, Or Related Field.

  • Minimum Of 2 Years Experience In Case Management, Preferably In Healthcare Or Insurance Industry.

  • Knowledge Of Health Care Systems And Regulations, Including Medicare And Medicaid.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Clients, Providers, And Interdisciplinary Teams.

  • Ability To Manage A Caseload, Prioritize Tasks, And Make Sound Decisions In A Fast-Paced And Changing Environment.
