  1. Provide high-quality care coordination services to individuals with intellectual and developmental disabilities (IDD) in accordance with UnitedHealth Group's standards and best practices.
  2. Conduct comprehensive assessments and develop individualized care plans for each client, with a focus on promoting overall health and well-being.
  3. Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure coordinated and holistic care for clients.
  4. Serve as a primary point of contact for clients and their families, providing education, support, and resources to help them navigate the healthcare system.
  5. Maintain thorough and accurate documentation of all client interactions and services provided in compliance with applicable regulations and company policies.
  6. Monitor and evaluate the effectiveness of care plans and make necessary adjustments to ensure optimal outcomes.
  7. Advocate for clients and their families by identifying and addressing any barriers to accessing healthcare services.
  8. Stay up-to-date on current healthcare trends and best practices related to IDD and social services to continuously improve the quality of care provided.
  9. Adhere to ethical and professional standards in all interactions with clients, their families, and team members.
  10. Participate in ongoing training and professional development opportunities to enhance skills and knowledge in the field of IDD and social services.
  11. Serve as a positive representative of UnitedHealth Group by upholding the company's mission, vision, and values in all interactions.
  12. Act as a leader and mentor to other team members, providing guidance and support as needed.
  13. Maintain a flexible schedule to accommodate client needs, including occasional evenings and weekends.
  14. Travel to client residences, community settings, and other locations as needed to provide in-person care and support.
  15. Perform other related duties as assigned by the supervisor.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field: A Minimum Of A Bachelor's Degree In Nursing Or A Related Field Is Typically Required For A Field Rn Health Social Services Coordinator/Idd Job At Unitedhealth Group.
