We are currently seeking a compassionate and dedicated Nurse Practitioner to join our HouseCalls team on a per diem basis. In this role, you will have the opportunity to provide in-home care to our patients, promoting wellness and preventing disease.   • Conduct in-home visits for patients enrolled in our HouseCalls program
  • Perform comprehensive health assessments and develop individualized care plans for patients
  • Administer necessary medical treatments and procedures, including medication management and wound care
  • Educate patients and their families on disease prevention, management, and self-care techniques
  • Communicate with patients' primary care physicians and specialists to coordinate care
  • Document all patient interactions and maintain accurate and up-to-date medical records
  • Collaborate with interdisciplinary team members to provide high-quality, patient-centered care
  • Stay current on healthcare industry developments and best practices in order to continuously improve patient care
  • Adhere to all company policies, procedures, and regulatory requirements
  • Maintain a professional and empathetic demeanor while interacting with patients and their families.

Job Qualifications
  • Graduate Degree In Nursing From An Accredited Program: A Nurse Practitioner Must Have Completed A Graduate Degree In Nursing, Such As A Master Of Science In Nursing (Msn) Or A Doctor Of Nursing Practice (Dnp) From An Accredited Program.

  • Current State Licensure As A Nurse Practitioner: Candidates Must Have A Current And Active Nurse Practitioner License In The State Where They Will Be Practicing. This License Must Be In Good Standing With No Restrictions Or Disciplinary Actions.

  • Minimum Of 2 Years Of Experience As A Nurse Practitioner: Unitedhealth Group Requires Candidates To Have A Minimum Of 2 Years Of Experience Working As A Nurse Practitioner In A Clinical Setting. This Experience Should Include Primary Care And/Or House Call Experience.

  • Excellent Clinical And Assessment Skills: The Nurse Practitioner Must Have Strong Clinical And Assessment Skills, Including The Ability To Evaluate And Diagnose Patients, Develop Treatment Plans, And Prescribe Medications. This Includes A Thorough Understanding Of Evidence-Based Practice And Clinical Guidelines.

  • Strong Communication And Interpersonal Skills: As A Nurse Practitioner For Unitedhealth Group's Housecalls Program, Candidates Must Have Excellent Communication And Interpersonal Skills. This Includes The Ability To Establish Rapport With Patients And Families, Collaborate With Other Healthcare Professionals, And Effectively Communicate Medical Information To Patients.
