  1. Serve as the primary point of contact for patients, healthcare providers, and other stakeholders involved in patient care.
  2. Coordinate and schedule appointments, procedures, and follow-up visits for patients.
  3. Communicate with patients and their families to provide information, answer questions, and address concerns regarding their care.
  4. Collaborate with healthcare providers to ensure timely and effective delivery of services to patients.
  5. Maintain accurate and up-to-date patient records, including medical history, treatment plans, and insurance information.
  6. Advocate for patients and assist with navigating the healthcare system to ensure they receive the best possible care.
  7. Monitor and track patient progress and outcomes, and communicate any changes or concerns to healthcare providers.
  8. Facilitate communication and coordination between different healthcare providers involved in a patient's care.
  9. Educate patients on their treatment plans, medications, and self-care techniques to promote optimal health outcomes.
  10. Remain knowledgeable about healthcare policies, procedures, and regulations to ensure compliance and patient safety.
  11. Use electronic medical records and other software systems to document and track patient care.
  12. Participate in team meetings and contribute ideas for improving patient care and processes.
  13. Collaborate with insurance companies and other third-party payers to ensure timely and accurate reimbursement for services.
  14. Provide exceptional customer service to patients and their families, addressing any concerns or issues in a timely and professional manner.
  15. Continuously seek opportunities for professional development and growth to enhance skills and knowledge related to patient care coordination.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Nursing, Or Related Field.

  • Minimum Of 3 Years Experience In A Healthcare Setting, Preferably In A Care Coordination Or Patient Advocacy Role.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Interact With Patients, Healthcare Providers, And Insurance Company Representatives.

  • In-Depth Knowledge Of Healthcare Laws, Regulations, And Policies, With A Focus On Patient Rights And Privacy.

  • Proficiency In Electronic Health Records And Other Healthcare Technology Systems.
