  1. Develop and implement individualized care plans for patients based on their unique needs and medical history.
  2. Coordinate and collaborate with healthcare providers, facilities, and community resources to ensure comprehensive and effective care for patients.
  3. Conduct thorough assessments of patient's medical, social, and psychological needs to identify any potential barriers to care and develop appropriate interventions.
  4. Monitor and evaluate patient progress and adjust care plans as needed to ensure optimal outcomes.
  5. Provide education and support to patients and their families regarding their medical conditions, treatment options, and self-management strategies.
  6. Maintain accurate and detailed documentation of patient care, including progress notes, treatment plans, and other relevant information.
  7. Communicate regularly with interdisciplinary team members to ensure continuity of care and identify any issues or concerns.
  8. Adhere to all regulatory and organizational guidelines, policies, and procedures related to patient care.
  9. Participate in case conferences and meetings to discuss patient care and collaborate with team members on effective treatment strategies.
  10. Serve as an advocate for patients, ensuring they receive high-quality and compassionate care at all times.
  11. Travel to various locations as needed to provide in-person care and support for patients.
  12. Stay current with industry trends and developments in healthcare to continuously improve patient care and outcomes.
  13. Maintain confidentiality and privacy of patient information at all times.

Job Qualifications
  • Registered Nurse (Rn) License In Good Standing With The State Board Of Nursing

  • Minimum Of 2 Years Of Clinical Experience, Preferably In A Case Management Or Home Health Setting

  • Strong Knowledge Of Medicare And Medicaid Regulations And Guidelines

  • Excellent Communication And Interpersonal Skills, With The Ability To Build Rapport With Patients And Healthcare Providers

  • Bachelor's Degree In Nursing Or Related Field, Or Equivalent Experience
