Your expertise and knowledge in the field of transplant pharmacotherapy will make a significant impact on the overall success of our transplant program.To be successful in this role, you must possess a Doctor of Pharmacy degree, be licensed to practice pharmacy in the state of Illinois, and have completed a specialized residency program in transplant pharmacotherapy.   1. Provide specialized pharmaceutical care to transplant patients, including medication management, education, and monitoring.
  2. Collaborate with interdisciplinary teams to develop and implement individualized treatment plans for transplant patients.
  3. Stay current on the latest research and advancements in transplant pharmacotherapy and incorporate them into patient care.
  4. Serve as a drug information resource for healthcare providers and patients regarding transplant medications.
  5. Monitor and assess transplant patients for drug interactions, adverse reactions, and therapeutic effectiveness.
  6. Conduct medication reconciliation and ensure accurate and safe prescribing practices for transplant patients.
  7. Participate in transplant rounds and provide recommendations for medication adjustments based on patient needs.
  8. Educate patients and their families regarding their medications, potential side effects, and proper administration techniques.
  9. Participate in research projects related to transplant pharmacotherapy and contribute to publications and presentations.
  10. Maintain accurate and up-to-date patient records and documentation in compliance with hospital and regulatory standards.
  11. Adhere to all hospital policies, procedures, and protocols related to medication management and patient care.
  12. Serve as a preceptor and mentor for pharmacy students, residents, and new staff members.
  13. Participate in quality improvement initiatives to enhance patient outcomes and safety.
  14. Collaborate with the pharmacy purchasing department to ensure adequate supply and availability of transplant medications.
  15. Maintain a professional and respectful demeanor with patients, colleagues, and other healthcare providers.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Of Pharmacy.

  • Completion Of A Post-Graduate Residency Program In Transplant Pharmacy Or Equivalent Experience.

  • Board Certification In Pharmacotherapy Or Specialty Certification In Transplant Pharmacy.

  • Minimum Of 3-5 Years Of Experience In A Clinical Pharmacy Role, With A Focus On Transplant Patients.

  • Strong Knowledge Of Transplant Pharmacotherapy, Immunosuppressive Medications, And Drug Interactions.
