
Nutrition Care Representative

- Greet patients and visitors in a warm and welcoming manner, and assist them with any questions or concerns they may have.
- Conduct patient assessments to determine individual nutritional needs and create personalized nutrition plans.
- Collaborate with healthcare professionals to ensure patients receive optimal nutrition care.
- Educate patients and their families on the importance of proper nutrition and provide resources and support for healthy eating habits.
- Monitor patient progress and adjust nutrition plans as needed to ensure optimal outcomes.
- Maintain accurate and detailed records of patient nutrition assessments, plans, and progress.
- Stay up-to-date on current nutrition research and trends to provide the most relevant and effective care.
- Work closely with food service staff to ensure patient meals meet dietary needs and preferences.
- Communicate effectively with patients, families, and healthcare team members to provide a seamless and positive patient experience.
- Participate in team meetings and training to continuously improve and enhance nutrition care services.
- Adhere to all hospital policies and procedures, as well as state and federal regulations, related to nutrition care.
- Maintain confidentiality of patient information and adhere to HIPAA regulations.
- Provide exceptional customer service and maintain a professional and compassionate demeanor at all times.
- Continuously strive to meet and exceed patient satisfaction goals.
- Other duties as assigned by supervisor or department leadership.
Bachelor's Degree In Nutrition, Dietetics, Or Related Field
Registered Dietitian (Rd) Or Registered Dietetic Technician (Dtr) Certification
Minimum Of 1-2 Years Of Experience In A Clinical Or Healthcare Setting
Excellent Communication And Interpersonal Skills To Effectively Interact With Patients, Families, And Healthcare Team
Knowledge Of Medical Terminology, Nutrition Assessment, And Nutrition Counseling Techniques
