  1. Perform accurate and thorough medication reconciliation for patients, including obtaining medication histories and updating medication lists.
  2. Collaborate with patients, caregivers, and healthcare providers to ensure accurate and complete medication information.
  3. Utilize electronic health record systems to document and track medication reconciliation processes.
  4. Identify and resolve discrepancies in medication lists and communicate any pertinent changes to appropriate healthcare team members.
  5. Maintain a strong understanding of medication terminology and patient care processes to ensure accurate reconciliation.
  6. Adhere to all hospital policies and procedures related to medication reconciliation and patient safety.
  7. Participate in ongoing education and training to maintain certification in medication reconciliation and stay current with industry standards and best practices.
  8. Act as a resource for other healthcare team members and provide guidance on proper medication reconciliation processes.
  9. Prioritize tasks and manage time effectively to meet daily medication reconciliation goals and deadlines.
  10. Maintain patient confidentiality and adhere to all HIPAA regulations during all aspects of the job.
  11. Collaborate with pharmacy staff to ensure timely and accurate medication delivery and administration.
  12. Proactively identify potential medication safety issues and report them to the appropriate parties for resolution.
  13. Maintain accurate documentation of all medication reconciliation activities and report any errors or discrepancies as necessary.
  14. Participate in quality improvement initiatives and make suggestions for process improvements to enhance patient safety and satisfaction.
  15. Act as a patient advocate and ensure that all medication reconciliation processes are carried out with the highest level of care and attention to detail.

Job Qualifications
  • Completion Of A Medication Reconciliation Technician Certification Program Approved By The American Society Of Health-System Pharmacists (Ashp) Or Other Recognized Accrediting Organization.

  • Minimum Of 1 Year Experience Working In A Healthcare Setting, Preferably In A Pharmacy Or Medication Reconciliation Role.

  • Strong Knowledge Of Medication Safety Practices And Ability To Accurately Reconcile Medications Across Multiple Settings (E.g. Inpatient, Outpatient, Long-Term Care).

  • Proficiency In Computer Skills And Experience Working With Electronic Health Record Systems.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Healthcare Providers, Patients, And Their Families To Ensure Accurate And Timely Medication Reconciliation.
