- Provide comprehensive and compassionate care to patients with heart failure, including assessment, diagnosis, treatment, and follow-up care.
- Collaborate with a multidisciplinary team of healthcare professionals to develop and implement individualized treatment plans for patients.
- Utilize advanced clinical knowledge and skills to manage complex and acute cases of heart failure.
- Educate patients and their families on heart failure management, including lifestyle modifications, medication management, and symptom recognition.
- Monitor and track patient progress, adjusting treatment plans as needed to ensure optimal outcomes.
- Maintain accurate and thorough patient records and documentation.
- Stay up-to-date with the latest developments in the field of cardiology and heart failure management.
- Participate in quality improvement initiatives and contribute to the overall success of the Heart Failure Program.
- Provide support and guidance to nursing and support staff, acting as a resource for any questions or concerns related to heart failure care.
- Adhere to all hospital policies, procedures, and protocols to ensure the highest level of patient safety and quality of care.
Current Licensure As An Advanced Practice Registered Nurse (Aprn) In The State Of New Hampshire.
Minimum Of 5 Years Of Experience As A Registered Nurse, With A Focus In Cardiology And Heart Failure Management.
Master's Degree In Nursing, With A Concentration In An Area Such As Adult-Gerontology Acute Care, Family Practice, Or Cardiology.
Certification As A Nurse Practitioner, With A Specialty In Cardiology Or Heart Failure Management, From A Nationally Recognized Organization Such As The American Nurses Credentialing Center (Ancc) Or The American Association Of Nurse Practitioners (Aanp).
Strong Clinical Skills And Knowledge In The Management And Treatment Of Heart Failure Patients, Including Experience With Medications, Diagnostic Testing, And Patient Education.
